HOW TO BECOME A PROVIDER |
Are you interested in becoming a provider for individuals who
participate in the Individual Options or Level One Waiver? If so, we
can help you through the process.
An individual or agency cannot provide Medicaid-funded services until certification is obtained from the Ohio Department of Mental Retardation and Developmental Disabilities.
To learn more about the process, or to get started with your application, contact Amy Smith or Steve Williams in our Provider Support and Development Office. They will be happy to walk you through the process and review your application before you submit it to ODMRDD. You may also visit the Ohio Department of MRDD website at www.odmrdd.state.oh.us to download the forms/rules and training requirements.
Remember, once you receive your certification, you must contact the BCBDD Office of Provider Support and Development before you can provide services. We will then add you to our list of Certified Providers.
